ACCIDENT REPORTING

There has been a steady trickle of reportable accidents and serious incidents that are unreported to the Air Accident Investigation Branch (AAIB). These are usually picked up by AAIB investigators who check the CAA Mandatory Occurrence Reports (MOR) scheme database for such lapses. In such cases, though, a lot of useful data or evidence is lost, and opportunities to prevent further accidents are missed.
