Quality has been an elusive goal throughout the software industry. Despite large amounts spent on program design, development, testing and maintenance, soft-ware has never reached the quality levels that are expected of computer hardware and many other manufactured products. Consequently, software defects are common and tolerated, regardless of the damage they may cause. The common assumption is that there simply is no cost-effective way to increase quality.
