Accidents happen in spite of the best training, mentoring, and leadership. Ship managers can do one of two things: they can accept that a certain level of mistakes is unavoidable but try to keep that level as low as they can; or they can analyse why errors of judgement occur, by whom, at what time of day, and which nationalities were involved. The first is irresponsible but, given pressure on time, it's understandable. The analytical option sounds wonderful, but how many companies have resource enough to create detailed monthly or quarterly reports?
