When planning mainte-nance jobs either by our own workforce or by contractors, it is important to consider all aspects of the work to be performed and the potential impact it may have on current operations. The following is an incident in another industry but from which much can be learned. It fortunately caused only material damages, but had the potential of injuring seriously both maintenance and operation personnel since it rup-tured the bottom of the vessel.
