NTSB: Amtrak's safety culture is 'failing'

After releasing a stinging report on the causes of a fatal 2016 Amtrak derailment, the National Transportation Safety Board (NTSB) last month issued 14 safety recommendations to the national passenger railroad, four rail labor organizations and the Federal Railroad Administration (FRA). The NTSB's report blamed the incident on Amtrak's weak safety culture. The derailment occurred April 3, 2016, in Chester, Pennsylvania, when Amtrak Train 89 traveling from Philadelphia to Washington, D.C., struck a backhoe, killing the backhoe operator and a track supervisor. Thirty-nine other people were injured.
