Just one month after National Transportation Safety Board (NTSB) Chairman Robert Sumwalt lambasted Amtrak for its "failing" safety culture, the railroad experienced yet another fatal accident on Dec. 18. During the incident, an Amtrak-operated train with 12 passenger cars and two locomotives derailed on its inaugural run across the newly built Point Defiance Bypass on the Cascades route between Seattle and Portland, Oregon. The derailment occurred on a bridge in DuPont, Washington.
