The world's worst industrial accident of the 1990s: what happened and what might have been: a quantitative study

The world's most destructive industrial accident of the current decade, in terms of human lives lost(over 60)and damage to property (over $15 million), occurred at the petroleum refinery of Hindustan Petroleum Corporation Limited(HPCL)at Vishakhapatnam, India, On September 14, 1997. In this paper we have used two computer Automated tools recently developed by us, MAXCRED[7], and DOMIFFECT[8], to determine their ability of forecast the details of Theincident.
