Someone working on a railroad crew in Graniteville, S.C., on a night in January 2005 left a switch in the wrong position. The mistake directed a freight train off the main line and onto a siding, where it struck a stationary train. Almost 20 cars derailed. One of them, a tank car carrying chlorine, ruptured.
