Information about failures during operation of complex, hazardous systems is invaluable for organizational learning and safety improvement. However, where the performance of practitioners, such as pilots, nurses and process plant operators, is concerned, many problems have been experienced in acquiring such data. A key reason is people's fear that reports they make about failures may lead to reprimands, other disciplinary action, or even legal proceedings. This problem is exacerbated by what many perceive as a trend to criminalization of human error. Just Culture is a response to these issues and focuses on the question of how to improve safety while at the same time satisfying demands for accountability. The book is of interest to all stakeholders in just culture, including researchers, practitioners, regulators, customers, members of the legal profession, and politicians.
