It wasn't that the City of Edmonton, Alberta, wasn't doing the right things in terms of fleet safety - it was that those efforts weren't being intentionally promoted to full potential. Several years ago, as the Fleet Services branch prepared for an external safety audit, change was needed. Lines of communication weren't clear. Front-line staff and leaders weren't feeling confident to act on safety concerns on their own. Opportunities to learn from past mistakes were diminished.
