Mitigating accidents through inquiry-based education

An accident is 'the consequence of events and conditions that lead to an unsafe situation resulting in personal injury and/or property damage'. This series of events and conditions is known as the 'Accident Chain'. The 'Accident Chain' is made up of six components: 1) Conditioning Factors 2) Human Factors 3) Unsafe Acts 4) Unsafe Conditions 5) The Accident 6) The Injury or Damage In an 'Accident Chain', The Accident' is the intersection of these components. Something or someone has to bring these components together in such a way to create the unsafe situation. We can however remove the 'Conditioning Factors' and/or 'Unsafe Conditions' to break the chain.
