Parathyroid Gland Preservation in situ versus Total Parathyroidectomy with Autotransplantation in Patients at Risk of MEN2A1

Dear Sir: In their series of 12 RET carriers operated on between 1994 and 2006, Yoshida et al. demonstrate the feasibility of total parathyroidectomy with autotransplantation of 3 normal-appearing parathyroid glands into the forearm as an alternative to in situ preservation of parathyroid glands [1]. Because each parathyroid cell inherited the RET mutation, there is obviously some risk that one of the cells may acquire those somatic mutations required for giving rise to "recurrent" parathyroid disease.
