Urine protein excretion is a prerequisite test for evaluation of living kidney donor candidates. According to Kidney Disease: Improving Global Out- comes (KDIGO) living kidney donor guidelines, initial evaluation of donor albuminuria (screening) should be performed using random urine albumin-to-creatinine ratio (ACR). 1 Donor albuminuria should be confirmed using albumin excretion rate (AER, mg/d) in a timed urine specimen. Urine AER <30 mg/d is considered an acceptable level for donation.
