How private is Talk Talk's 'Advanced Care'?

My landline with TalkTalk failed the other day. TalkTalk has now fixed this, but the assistant I spoke to recommended I sign up to the company's 'Advanced Care' scheme, which offers help for people in need. It means TalkTalk can hand my data to third parties, with or without my consent. What privacy issues does this raise? Privacy law is a minefield, but as long as TalkTalk is clear about what data it wants and why, it's probably doing nothing wrong. Its 'Advanced Care' service appears to actually be called TalkTalk Accessibility . It offers help to disabled people, dividing its assistance into visual, hearing, cognitive and mobility (pictured left).
