Early Tuesday morning, Randy, the human resources manager for XYZ Corporation, answers a phone call from Marlene, the production manager. Marlene relays the bad news—another accident has occurred in the shop. A material handler twisted his ankle when he tripped over a pallet of material staged in a pedestrian aisle. Although not seriously injured, the material handler required medical attention. Randy slowly shakes his head as he reviews the recordable injury trend for the past three months. The injury rate has nearly tripled compared to the prior year, and trip-and-fall injuries top the list.
