COPD患者血浆可溶性髓样细胞触发性受体-1、白三烯B4测定及临床意义

         

摘要

目的:探讨可溶性髓样细胞触发受体-1(soluble triggering receptor expressed on myeloid cells-1, sTREM-1)、白三烯B4(LeukotrieneB4,LTB4)在COPD患者炎症反应中的作用及临床意义。方法根据GOLD指南对所有患者综合评估后分组,收集各组患者及对照者外周血标本,用ELISA法测定标本中的sTREM-1、BLT4浓度。结果①COPD患者A、B、C、D各组及健康对照组中血浆sTREM-1浓度分别为(2.41±0.68) ng/mL、(4.33±0.83) ng/mL、(5.23±0.88) ng/mL、(8.44±0.69)ng/mL和(1.11±0.20)ng/mL, A、B、C、D组各组与健康对照组及各组间比较,差异均具有统计学意义(P<0.01)。②COPD患者sTREM-1浓度与FEV1占预计值%负相关(r=-0.675, P<0.01)。③COPD患者A、B、C、D各组及健康对照组LTB4浓度分别为(1.68±0.46)ng/mL、(1.96±0.12)ng/mL、(2.24±0.98)ng/mL、(2.52±0.15)ng/mL、(1.36±0.19)ng/mL, A、B、C、D组与健康对照组及各组间比较,差异均有统计学意义(P<0.01)。④COPD患者LTB4浓度与FEV1占预计值%负相关(r=-0.848,P<0.01)。⑤COPD各组中sTREM-1浓度与LTB4浓度成正相关(r=0.872, P<0.01)。结论①随着COPD分组病情加重,sTREM-1、LTB4浓度逐渐升高,二者参与肺功能的损害,可能是COPD炎症反应的生物标记物。②COPD各组中sTREM-1浓度与LTB4浓度正相关,二者可能在同一信号传递通路上,相互作用促进炎症反应。%Objective To test the concentration of sTREM-1 and LTB4 in peripheral blood plasma from patients with dif-ferent groups of COPD, and to analysis the correlation of sTREM-1、LTB4 and lung function. And explore the implication for the assessment of the inflammation of COPD. Methods According the Global strategy for the diagnosis, management, and prevention of chronic obstructive pulmonary disease (Revised 2011) the patients were divided into four groups. The peripheral blood of pa-tients and healthy controls were collected, and then measured the concentration of sTREM-1 by ELISA method. Results ①The concentration of sTREM-1 from the COPD patients of A、B、C、D groups and healthy control groups were respectively as ( 2. 41 ± 0. 68)ng/mL、(4. 33 ± 0. 83) ng/mL、(5. 23 ± 0. 88) ng/mL、(8. 44 ± 0. 69) ng/mL and (1. 11 ± 0. 20) ng/mL. A contrast the concentration of sTREM-1 from the COPD patients each group and the healthy control group, a significant difference between each group of COPD and healthy control (P<0. 01). ②The concentration of sTREM-1 from the COPD patients were negative correlated with FEV1% predicted (r= -0. 675, P<0. 01). ③The concentration of LTB4 from the COPD patients of A、B、C、D groups and healthy control groups were respectively as ( 1. 68 ± 0. 46 ) ng/mL、( 1. 96 ± 0. 12 ) ng/mL、( 2. 24 ± 0. 98 ) ng/mL、( 2. 52 ± 0. 15 ) ng/mL and(1. 36 ± 0. 19)ng/mL. A contrast the concentration of LTB4 from the COPD patients each group and the healthy control group, a significant difference between each group of COPD and healthy control ( P<0. 01 ) .④The concentration of LTB4 from the COPD patients were negative correlated with FEV1% predicted (r= -0. 848,P<0. 01). ⑤The correlation of the concentra-tion of sTREM-1 and LTB4 was positive(r=0. 872, P<0. 01). Conclusion ①As the disease in patients with COPD exacerba-tion the concentration of sTREM-1and LTB4 increased gradually. Both sTREM-1and LTB4 involve in lung function damage; they may become biomarker and assess the disease among different groups of COPD. ②The concentration of sTREM-1 and LTB4 is pos-itive correlation among different groups of COPD, both may be on the same signaling pathway and promote inflammation mutually.
