3.0T磁共振成像Tissue4D灌注成像与弥散成像定量评估前列腺癌特性及肿瘤等级的可行性探讨

     

摘要

Objective To quantitatively analyze the characteristics of prostate cancer by the Ktrans value of Tissue4D perfusion imaging and the ADC value of DWI ,and to investigate the correlation between Ktrans ,ADC value and Gleason (GS) grade ,PSA and fPSA/PSA ratio of prostate cancer ,and the feasibility of quantitative e-valuation of the classification of prostate cancer .Methods According to pathological results , Ktrans and ADC values of nodal regions of interest were measured . According to the Gleason score , the patients with prostate cancer were divided into 3 groups , which were low grade malignant group (GS3+3) ,moderate malignant group (GS3+4) ,and highly malignant group (GS4+3 ,3+5 ,4+4 ,4+5 ,5+4).Spearman correlation test was used to preliminarily study the correlation between Ktrans value ,ADC value ,GS score ,PSA and fPSA/PSA ratio of prostate cancer .Results A significant negative correlation between Ktrans value and ADC value (r=-0.575 ,P<0.01) was observed in prostate cancer nodules ,and also between Ktrans value and fPSA/tPSA ,(r=-0.374) . While a positive correlation between Ktrans value and GS grading (r=0.201) was observed . There was no significant correlation between Ktrans value and PSA .In addition , ADC was negatively related to GS grading (r=-0.632) and PSA (r=-0.658) ,and displayed a positive correlation with fPSA/tPSA (r=-0.312).The Ktrans values of the low malignant group and of the high malignant group showed a significant difference (P=0.002).There was no statistical significance between the Ktrans values of the middle malignant group and the other two groups .Conclusion There is a correlation between Ktrans value ,ADC value ,and GS grading for pros-tate cancer .They are important imaging indexes for the quantitive evaluation of prostate cancer characteristics and tumor grade.%目的 采用Tissue4D灌注成像的Ktrans值及弥散成像(DWI)的表观扩散系数(ADC)值定量分析前列腺癌特性,初步探讨Ktrans值、ADC值与前列腺癌Gleason(GS)分级、前列腺特异性抗原(tPSA)、游离前列腺特异性抗原(fPSA)/tPSA比值的相关性,及定量评估前列腺癌肿瘤分级的可行性.方法 依据病理结果提示,选取癌结节感兴趣区测量Ktrans值及ADC值.根据Gleason评分结果将前列腺癌分为3个级别组,分别为低度恶性组(GS3+3) 、中度恶性组(GS3+4) 、高度恶性组(GS4+3 ,3+5 ,4+4 ,4+5 ,5+4) .结果 本组研究中前列腺癌结节Ktrans值与 ADC值呈负相关(r=- 0.575 ,P< 0.01). Ktrans值与 GS 分级呈正相关,r=0.201 ,与fPSA/tPSA呈负相关(r=-0.374) ;Ktrans值与PSA之间无相关性. ADC值与GS分级呈负相关(r=-0.632) ;与PSA呈负相关(r=-0.658) ;与fPSA/tPSA呈正相关(r=0.312).而低、中、高度恶性组3组之间Ktrans值低度和高度恶性组比较差异有统计学意义(P=0.002).而中度恶性组间Ktrans值差异较大,其均值与低度恶性组及高度恶性组之间的差异无统计学意义.结论 Ktrans值、ADC值与前列腺癌的GS分级存在相关性,是定量评估前列腺癌特性及肿瘤等级的重要影像学指标之一.
