PCOS 患者血清 FABP4水平与糖脂代谢紊乱的相关性

摘要

目的：探讨多囊卵巢综合征（PCOS）患者血清脂肪型脂肪酸结合蛋白4（FABP4）水平与糖脂代谢紊乱的相关性。方法采用ELISA法检测76例PCOS患者（ PCOS组，其中肥胖者50例，非肥胖者26例）及40例健康志愿者（对照组）血清FABP4水平。检测两组血清TC、TG、HDL、LDH、载脂蛋白A1、B（ ApoA1、ApoB）水平、计算胰岛素抵抗指数（HOMA-IR）；行糖耐量试验，采用氧化酶法测定血清葡萄糖水平（BG 0 h、1 h、2 h、3h），放射免疫法测定血清胰岛素水平（ INS 0 h、1 h、2 h、3 h）。分析血清FABP4水平与血糖、血脂指标的相关性。结果 PCOS组血清FABP4水平高于对照组，且肥胖PCOS患者高于非肥胖患者（P均＜0．05）。 PCOS组TC、TG、LDL、BG 0 h、INS 0 h、HOMA-IR高于对照组（P均＜0．05）。肥胖者TC、TG、LDL、ApoB、BG 0 h、BG 1 h、BG 2 h、INS 0 h、HOMA-IR高于非肥胖者（P均＜0．05）。 PCOS组血清FABP4水平与TG、TC、ApoB、LDL、HOMA-IR、INS 0 h、INS 2 h呈正相关（r分别为0．35、0．33、0．26、0．25、0．36、0．32、0．32，P均＜0．05）。结论 PCOS患者血清FABP4水平升高，其与PCOS糖脂代谢紊乱的发生有关。%Objective To study the relationship between serum FABP 4 and metabolic disorders in women with poly-cystic ovarian syndrome(PCOS).Methods Serum FABP4 levels in 76 PCOS women (consist of 50 obesity, 26 non obese) and 40 healthy volunteers as control group were detected by enzyme-linked immunosorbent assay ( ELISA) method. Serum lipid (TC,TG,HDL,LDH,ApoA1,ApoB) were observed.HOMA-IR was calculated.Oral glucose tolerance test ( OGTT) were conducted to test serum glucose level ( BG 0 h, 1 h, 2 h, 3 h) by oxidase method .The level of serum insu-lin radioimmunoassay(INS 0 h,1 h,2 h,3 h)was determined.The correlation of serum FABP4 levels and markers of meta-bolic disorders were analyzed .Results Serum FABP4 levels of PCOS groups were significantly higher than control groups , in which obese PCOS groups were significantly higher than that of non-obese PCOS groups(all P<0.05).Compared with control groups, the PCOS women had significantly higer TC,TG,LDL,BG 0 h,INS 0 h,HOMA-IR(all P<0.05).Com-pared with non-obese PCOS women, the obese PCOS women had significantly higer TC ,TG,LDL,ApoB,BG 0 h,BG 1 h, BG 2 h,INS 0 h,HOMA-IR(all P<0.05).Correlation analysis showed that serum FABP4 levels were positively correlated with TG, TC, ApoB, LDL, HOMA-IR, INS 0 h, INS 2 h(r was 0.35, 0.33, 0.26, 0.25, 0.36, 0.32, 0.32, respec-tively, all P<0.05).Conclusions PCOS women had high level of serum FABP 4 which had relationship with metabolic disorder in glucose and lipid .
