乳腺癌的全数字化乳腺摄影表现与癌基因表达的相关性研究

         

摘要

目的:探讨乳腺癌全数字化乳腺摄影(FFDM)表现与C-erbB-2、ER、PR表达之间的相关性.方法:回顾性分析100例经手术病理证实为乳腺癌患者的FFDM表现,患者术前皆未行化疗或放疗,术后标本行免疫组织化学染色测定肿瘤细胞C-erbB-2、ER、PR的表达情况,并分析其与相应病灶FFDM表现的关系.结果:乳腺癌的FFDM主要征象表现为边缘形态不规则呈浅分叶或毛刺状边缘的肿块,边界模糊,瘤内或瘤旁有成簇细砂砾样钙化.乳腺癌病灶的大小、微钙化与C-erbB-2阳性表达呈正相关(P<0.01),毛刺状边缘与ER阳性表达呈正相关(P<0.05).结论:乳腺癌的FFDM表现与分子生物学指标C-erbB-2、ER之间存在一定的相关性.乳腺癌的FFDM表现在一定程度上反映了乳腺癌细胞的生物学行为,对乳腺癌的预后评估具有重要的价值.%Objective: To investigate the correlation between full-field digital mammography morphological features and C-erbB-2 , ER,PR expressions of breast cancer. Methods: Data of one hundred patients with breast cancer who received preoperative FFDM and radical mastectomy were analyzed retrospectively. The relationship between mammographic morphological features and C-erbB-2, ER, PR expressions of the tumor samples detected by immunohistochemistry was analysed. None of the patients received adjuvant chemotherapy or radiotherapy before operation. Results: Masses with rough boundaries,spiculate or shallow sublobe margin,fine sand-like calcification inside or beside the tumors were the main signs in FFDM. Tumor size and microcalcification were positively correlated with C-erbB-2 positive expression. Spiculate margin was positively correlated with ER positive expression. Conclusion : There are positive correlation between FFDM and C-erbB-2 , ER expressions of breast cancer. FFDM morphological features can be an indirect detection of biological behavior of breast tumor which is important for diagnosis and prognosis of breast cancer.
