SPGR-T2*WI鉴别颅内出血和钙化的定量分析

         

摘要

目的:研究扰相梯度双回波T2*加权成像(SPGR-T2*WI)序列诊断与鉴别诊断颅内出血和钙化的定量分析指标.方法:以多回波采集重度T2*WI三维梯度回波序列(ESWAN)作为金标准,对已确诊脑出血的18例38个病灶和颅内钙化7例11个病灶进行磁共振SPGR-T2*WI (15/30ms)检查.测量不同病灶T2*值,进行统计学分析比较.结果:SPGR-T2*WI序列对于血肿的发现率为CT的1.9倍,对微出血及脑干、小脑等部位出血SPGR-T2*WI比CT检查更有优势.出血(核心区)与钙化的T2*值有明显的统计学差异(P<0.05),但是出血(外周区)与钙化的T2*值无明显的统计学差异(P>0.05).结论:SPGR-T2*WI序列上颅内出血和钙化T2*值的定量分析,为实现脑血管病的MR一站式诊断奠定了基础.%Objective:The purpose of this study was to investigate the feasibility of the quantitative criterion in differrnential diagnosis of cerebral hemorrhage and calcification by using SPGR T2* WI sequence of MR. Methods:ESWAN takenrnas "golden" standard,MR SPGR-T2* WI with TE of 15ms and 30ms was performed in 18 cases (38 lesions) of cerebral hernmatoma and 7 cases (11 lesions) of calcification. The T2* value of the same dark area was measured at different TEs. Thernimaging findings were compared with SPGR-T2* Wl and CT. Results:The detecting rate of SPGR-T2* WI of cerebral hemrnorrhage was 1.9 times the rate of CT. Microbleeds and the cerebral hematoma which located in the brain stem and cerebelrnlum were easily found by using SPGR-T2* WI than CT. The T2* value at different TE values was statistically different atrnthe hematoma (core area) and calcification (P<0.05) of group cerebral hemorrhage. The T2 * value at different TE valuesrnwas not statistically different between group of cerebral hemorrhage (peripheral area) and calcification (P>0.05). Conclu-rnsion:The quantitative analysis of acute cerebral hematoma and calcification is available by measuring the T2* value at differrnent TE values. It would do contribute to a one shot diagnosis of cerebrovascular disease.
