中老年人轻度肾功能减退与血脂异常

摘要

Objective To explore the characteristic of serum lipid in middle and old age patients with mild renal insufficiency. Methods 266 cases, aged 45-75 years, were divided into 3 groups according to eGFR, and their serum lipid profile and related factors were analyzed. Influence of age on renal function parameters and serum lipid profile were adjusted by ANCOVA. Results Among the three groups,the level of LDL-C in the normal eGFR group was lower and the level of HDL-C was higher than that in the other two groups ( P ＜0. 05 or P ＜0. 01 ). The level of ApoA in eGFR moderately declined group was significantly lower than that in slightly declined group[ ( 1.14 ±0. 25) g/L vs ( 1.30 ±0. 26) g/L,P ＜0. 01 ]. Conclusions There were dyslipidemia, decreased HDL-C and ApoA, and increased LDL-C, in middle and old age patients with mild renal insufficiency, which may be related to the progress of renal dysfunction and poor prognosis.%目的 探讨中老年人轻度肾功能减退者的血脂特征.方法 将266例中老年人按照估测肾小球滤过率(eGFR)水平分为正常、轻度降低和中度降低3组;方差分析各组之间血脂以及相关生化指标的差异;多变量协方差分析校正年龄对肾功能和血脂的影响.结果 3组中低密度脂蛋白胆固醉(LDL-C)在eGFR正常组最低(P＜0.05),高密度脂蛋白胆固醉(HDL-C)在eGFR正常组最高(P＜0.01).载脂蛋白A(ApoA)在eGFR中度降低组比轻度降低组显著降低[(1.14±0.25)g/L比(1.30±0.26)g/L,P＜0.01].结论 轻度肾功能减退中老年人存在血脂代谢异常,以HDL-C和ApoA降低、LDL-C升高为主要表现;血脂异常可能与肾功能减退的进展和不良预后有关.
