老年糖尿病肾病病人口腔牙周健康状况及炎性因子水平调查研究

摘要

目的 调查老年糖尿病肾病病人口腔牙周健康状况及炎性因子水平. 方法 以46例老年糖尿病肾病病人为糖尿病肾病组,40例老年2型糖尿病病人为糖尿病组,另选择体检健康的老年人39例为对照组.3组均接受口腔牙周健康状况检查及外周血、龈沟液白介素-1β(IL-lβ)、IL-6及肿瘤坏死因子-α(TNF-α)水平检测.结果 (1)糖尿病肾病组牙周炎患病率为93.48％(43/46),明显高于糖尿病组的57.50％(23/40)及对照组的28.21％(11/39),差异均有统计学意义(P均≤0.05).糖尿病肾病组龈沟出血指数(SBI)、牙齿填充指数(DMFT)、简化口腔卫生指数(OHI-S)、牙周袋探诊深度(PD)、临床附着丧失(CAL)、牙松动度明显高于糖尿病组及对照组(P均≤0.05).(2)糖尿病肾病组外周血及龈沟液IL-1β、IL-6及TNF-α水平均明显高于糖尿病组及对照组(P均≤0.05). 结论 老年糖尿病肾病病人口腔健康状况较差,牙周炎患病率及炎性因子水平较高.%Objective To study the prevalence of periodontal disease and the level of inflammatory factors in elderly patients with diabetic nephropathy.Methods forty-six elderly patients with diabetic nephropathy were choosed as diabetic nephropathy group,40 elderly patients with type 2 diabetes were selected as diabetes group,and 39 healthy old people were selected as control group.The oral and periodontal health status were surveyed,and the the levels of interleukin-1β (IL-I[3),IL-6,tumor necrosis factor(TNF-a) in peripheral blood and fluid of gingiva groove were detected in three groups.Results (1)The prevalence rate of periodontitis was 93.48％ (43/46) in diabetic nephropathy group,and was significantly higher than that in diabetes group (57.50％,23/40) and control group (28.21％,11/39),the differences between three groups were statistically significant (P≤0.05).Gingival sulcus bleeding index (SBI),teeth filling index (DMFT),simplify oral hygiene index (OHI-S),periodontal pocket probing depth (PD),clinical attachment loss (CAL),tooth mobility in diabetic nephropathy group were significantly higher than those in diabetes group and control group (P≤0.05).(2)The levels of IL-1β,IL-6,TNF-α of peripheral blood and fluid of gingiva groove in diabetic nephropathy group were significantly higher than those in diabetes group and control group (P≤0.05).Conclusions The oral health status are worse,the prevalence of periodontitis and the level of inflammatory factors are higher in elderly patients with diabetic nephropathy.
