原发性肝癌经导管肝动脉化疗栓塞术后患者家属照护需求的相关因素分析

摘要

Objective:To investigate the status of family care needs of patients with primary liver cancer after transcatheter arterial chemoembolization (TACE),and analyze the related influence factors of the family care needs for postoperative patients.Methods:A total of 120 family members of patients with primary liver cancer who received TACE in our hospital from January 2014 to December 2016 were selected as the subjects investigated.According to the critical care family needs inventory (CCFNI),a questionnaire survey was conducted to investigate the status of family care needs of 120 patients.Logistic regression analysis was adopted to analyze the related influence factors of family care needs.Results:The scores of the CCFNI of 120 family members of patients were (135.80 ± 13.58)with 86 cases of strong needs,accounting for 71.67％.Logistic multiple factor regression analysis showed that educational level,medical payment method,disease control and accompanying experience were independent influence factors of family care needs(P ＜ 0.05).Conclusion:A majority of family members have care needs for patients with hepatocellular carcinoma after TACE.Their needs and desire are related with educational level,medical payment method,disease control and accompanying experience.The improved medical security system and targeted mental health education are helpful to improve the psychological status of family members of patients and reduce care needs.%目的:调查原发性肝癌经导管肝动脉化疗栓塞(TACE)术后患者家属照护需求现状,分析影响术后患者家属照护需求的相关因素.方法:选择2014年1月～2016年12月于我院接受TACE术的原发性肝癌患者家属120名作为研究对象.根据危重患者家属需求量表(CCFNI),采用问卷调查的形式,对120名患者家属照护需求现状进行调查.采用logistic多因素回归分析影响家属照护需求的相关因素.结果:120名患者家属CCFNI照护需求评分为(135.80±13.58)分,需求强烈者86例,占71.67％.Logistic多因素分析显示文化程度、医疗支付方式、病情控制及陪护经历是影响家属照护需求的独立因素(P＜0.05).结论:肝癌TACE术后患者家属多有照护需求,其需求愿望与文化程度、医疗支付方式、病情控制及陪护经历有关,完善医疗保障体系,针对性进行心理健康教育,有助于改善患者家属心理状况,降低照护需求.
