阴道分娩产后出血患者相关高危因素的调查分析

Objective:To investigate the high risk factors related to postpartum hemorrhage in patients with vaginal delivery and the interventions for high-risk factors were studied. Methods:A total of 240 cases of vaginal delivery patients who were hospitalized in our hospital from April 2014 to June 2017 were randomly divided into observation group and control group. Postpartum hemorrhage patients with vaginal delivery were selected as the observation group, while the vaginal delivery patients without postpartum hemorrhage were selected as the control group. The causes of postpartum hemorrhage were analyzed and compared. Results: Patients with anemia, advanced age, miscarriage more than 2 times, lower education level, hypertension, and longer duration of the third stage of labor affected the postpartum hemorrhage in patients with vaginal delivery. Multivariate logistic regression analysis found that in addition to gestational diabetes mellitus, the number of abortions, number of births, number of pregnancies, age, body mass index, placental factors, third stage of la-bor, hypertension during pregnancy, education level, soft birth canal laceration, anemia, scar uterus, use of tocolytic agents and macrosomia were the high risk factors for postpartum hemorrhage after vaginal delivery(P<0. 05). Conclusion: The investigation and analysis of related high risk factors of postpar-tum hemorrhage in vaginal delivery and interventions for high risk factors are of great significance to patients.%目的:对阴道分娩产后出血患者相关高危因素进行调查,针对高危因素的干预措施进行研讨.方法:将2014年4月至2017年6月在我院住院治疗的240例经阴道分娩患者随机等分为观察组和对照组,经阴道分娩产后出血的患者作为观察组,经阴道分娩非产后出血患者作为对照组,分析影响患者产后出血的原因,将原因进行对比分析.结果:患者贫血、高龄、流产2次以上、文化程度较低、高血压、第三产程时间较长等因素均对阴道分娩产后出血患者有影响;经多因素logistic回归分析发现,除妊娠期糖尿病外,流产次数、生产次数、怀孕次数、年龄、体重指数、胎盘因素、第三产程、妊娠期高血压、文化程度、软产道裂伤、贫血、瘢痕子宫、宫缩抑制剂的使用、巨大儿均是阴道分娩产后出血的高危因素(P<0.05).结论:阴道分娩产后出血患者相关高危因素的调查分析与针对高危因素的干预措施对患者具有重要意义.
