Objective It is to investigate the method and effect of endoscopic mucosal resection in the treatment of colorectal adenoma. Methods 43 patients with colorectal adenoma, who diagnosed by endoscope, and without surgical contraindications, and lesions from mucous layer or submucosal layer, were selected to receive endoscopic mucosal resection surgery. Lesion sample was fixed and received pathology examination. Results 43 patients with colorectal adenoma were completely removed. The average time-consuming of endoscopic mucosal resection was ( 37.6±4.5 ) minutes. Specimen pathology results :23 cases were tubular adenoma, 11 cases were mixed adenoma and 9 cases were villous adenoma. Postoperative acute bleeding was occurred in 1 case and delayed bleeding were occurred in 2 patients, and the bleeding were stopped after appropriate hemostasis. All patients were follow-up 6 months, the perforations or stenoses were not occurred. Conclusion Endoscopic mucosal resection is important methods in the treatment of colorectal adenomas disease , the operation technique is simple , less incidence,more efficient and worthy of clinical application.%目的 探讨内镜下黏膜切除结直肠腺瘤的方法和疗效.方法 选择经内镜检查发现结直肠腺瘤病变43例,并明确病变源于黏膜层或黏膜下层,确定患者无手术禁忌证后,择期选择内镜下黏膜切除术(EMR)切除病变.病变标本固定、标注后送病理检查.结果 所有43例结直肠腺瘤病变被完全切除,内镜下黏膜切除术耗时为23～85(37.6±4.5)min.标本病理结果为:管状腺瘤23例,混合型腺瘤11例,绒毛状腺瘤9例.术后急性出血1例,延迟出血2例,予相应止血治疗后出血停止.所有患者术后随访12个月,无穿孔或管腔狭窄出现.结论 内镜下黏膜切除术是临床上治疗结直肠腺瘤病变的重要方法,操作技术简单,严重并发症少见,疗效理想,值得临床推广.
