普伐他汀对血脂正常高血压患者颈动脉内膜、踝臂指数和高敏C反应蛋白的影响

摘要

目的 观察普伐他汀对血脂正常高血压患者颈动脉内膜-中膜厚度(IMT)、踝臂指数和高敏C反应蛋白(hs-CRP)的影响.方法 选取血脂正常的高血压患者120例,随机分为观察组和对照组,每组60例.对照组予以常规的降压药物治疗,而观察组在对照组治疗的基础上予以普伐他汀治疗.观察2组治疗前后的收缩压(SBP)、舒张压(DBP)、脉压(PP)、踝臂指数、颈动脉IMT值和血浆hs-CRP水平.结果 治疗后,2组SBP、DBP和hs-CRP水平均较治疗前明显降低,观察组的SBP和hs-CRP水平较对照组降低更为明显,而观察组的PP水平和颈动脉IMT值较治疗前和对照组明显降低.2组踝臂指数较治疗前明显升高,而观察组较对照组升高更为明显.2组治疗均能耐受,观察组的不良反应发生率为12%,对照组为10%,2组比较无显著性差异(P>0.05).结论 对于血脂正常的高血压患者,普伐他汀可以改善动脉内膜功能和炎症反应,并且具有辅助降压作用.%Objective It is to observe the effects of pravastatin on carotid artery intima-media thickness ( IMT ), ankle -brachial index, and high-sensitive C - reactive protein( hs - CRP ) in patients with hypertension and normal blood lipids. Methods 120 patients with hypertension and normal blood lipids were randomly divided into observation group and control group, each group had 60 cases. The control group was given conventional antihypertensive drug therapy, the observation group was given pravastatin on the basis of the control group. The systolic blood pressure ( SBP ), diastolic blood pressure ( DBP ), pulse pressure ( PP ), ankle-brachial index, carotid IMT values and plasma hs - CRP levels were observed before and after treatment in both groups. Results After treatment, the levels of SBP, DBP and hs - CRP in both groups were significantly lower than those before treatment, the levels of SBP and hs - CRP in observation group were decreased more significantly , while the levels of PP and carotid IMT in observation group decreased significantly compared with that before treatment and in the control group. Ankle-brachial indexes were significantly higher in both groups than those before treatment, while the levels of observation group increased more significantly compared with the control group. The rate of adverse reaction was 12% in observation group and 10% in control group, there was no significant difference between the two groups ( P >0. 05 ). Conclusion Pravastatin can improve arterial intimal function and inflammatory response in patients with hypertension and normal blood lipids and has assisted antihypertensive effect.
