孟鲁司特钠联合糖皮质激素对中重度哮喘患儿肺功能及IgE的影响

摘要

目的 研究孟鲁司特钠联合糖皮质激素对中重度哮喘患儿肺功能及IgE的影响.方法 中重度哮喘患儿54例按入院顺序随机分为治疗组与对照组各27例.对照组予单纯吸入布地奈德气雾剂100μg/次,早晚各1次.治疗组在对照组基础上加用孟鲁司特钠片口服,2~6岁4mg/次,7~14岁5mg/次,均每晚1次.2组总疗程均为12周.结果 ①治疗组总有效率显著高于对照组(P<0.01).②治疗后2组患儿FEV1%、PEF%均较治疗前显著升高,且治疗组升高幅度显著高于对照组(P<0.01).③治疗后2组IgE均较治疗前显著下降,治疗组下降幅度均比对照组更显著(P<0.01).结论 吸入糖皮质激素联合口服孟鲁司特钠片治疗中重度哮喘患儿,可从不同途径抑制气道炎症,降低气道高反应性,改善肺功能,缓解临床症状,且易于接受,适合推广.%Objective It is to study the influence of montelukast sodium plus glucocorticoid on lung function and IgE in moderate and severe asthma children. Methods 54 cases of moderate-severe asthma children were divided into two groups with 27 cases in each group according to the hospitalized sequence. The control group was only given budesonide aerosol 100 ug once time in the morning and at night. The treatment group was given oral montelukast on the basis of therapy in the control group, the ones of 2 - 6 years old 5mg per time at night, and that of 7 - 14 years old 5mg per time at night. The total course was 12 weeks in both groups. Results The total effective rate of treatment group was higher than that of control group( P < 0. 01 ). FEV1% ,PEF% of the two groups after treatment were increased significantly, and the treatment group showed bigger range( P < 0.01 or P < 0. 001 ). IgE of the two groups were decreased after treatment, and the treatment group showed bigger range( P <0.001 ). Conclusion Montelukast sodium plus glucocorticoid could suppress the inflammation in airway, reduce the high reactivity in airway, improve the lung function, relieve clinical symptoms from manyway. It' s easy to be applied and appropriate to be popularized.
