益气活血法干预糖尿病动脉弹性减退临床观察

Objective It is to observe the clinical therapeutic effect of benefiting vital energy and activating blood circulation on decreased arterial elasticity in diabetic patients.Methods 58 diabetic cases with Qi asthenia and blood stasis type were selected.30 cases of them were randomly applied with the traditional Chinese medicine with the main therapeutic principle of benefiting vital energy and activating blood circulation.The other 28 cases were applied with aspirin as a positive control.The course of treatment was 8 weeks.The indexes of arterial elasticity were observed before and after the treatment.Results In the test group,the pressure-strain elastic modulus (Ep),stiffness parameter β,pulse wave velocity β (PWVβ) and augmentation index (AI) of carotid arteries after treatment were lower than those before treatment.But there're no significant differences between them.The PWVβ and AI of femoral arteries and the right ABI after treatment in the test group were significantly lower than those before treatment (P ＜ 0.05).While the corresponding indexes in the control group had no significant differences.Conclusion The elasticity of main arteries in diabetic patients with Qi asthenia and blood stasis type can be effectively improved by the traditional Chinese medicine with the main therapeutic principle of benefiting vital energy and activating blood circulation.Such kind of traditional Chinese medicine is hoped to be an effective therapeutic method for preventing and treating diabetic arteriosclerosis diseases in their early stage.%目的 观察应用益气活血法为主的中药干预糖尿病患者动脉弹性减退的临床效果.方法 选取气虚血瘀型糖尿病患者58例,随机分为试验组30例和对照组28例,试验组运用益气活血法为主中药干预治疗,对照组采用阿司匹林治疗,疗程均为8周,观察2组治疗前后动脉弹性指标的变化.结果 试验组治疗后颈动脉压力-应变弹性系数(Ep)、硬化参数(β)、脉搏波传导速度(PWVβ)、管径增大指数(AI)均较治疗前有所下降,但无显著性差异(P均＞0.05).试验组治疗后股动脉PWVβ、AI较治疗前明显下降(P均＜0.05).试验组右侧踝臂指数(ABI)较治疗前显著下降(P＜0.05),对照组无明显差异.结论 益气活血法为主的中药治疗可有效改善气虚血瘀型糖尿病患者的大动脉弹性,有望成为早期防治糖尿病动脉硬化性病变的有效治疗手段.
