丹红注射液联合参松养心胶囊治疗心血瘀阻证不稳定型心绞痛疗效及对血管内皮功能和炎症因子的影响

摘要

目的 探讨丹红注射液与参松养心胶囊联合治疗心血瘀阻证不稳定型心绞痛(UAP)的临床效果及对患者血管内皮功能和炎症因子的影响.方法 将110例心血瘀阻证UAP患者随机分为对照组55例和观察组55例,2组均给予常规西医基础治疗,对照组同时给予丹红注射液静滴,观察组在对照组治疗基础上给予参松养心胶囊口服.治疗2周后,评价2组心绞痛疗效和心电图疗效,比较2组治疗前后肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、内皮素(ET)、血栓素B2(TXB2)、一氧化氮(NO)、全血黏度(WBV)、红细胞压积(HCT)、血沉(ESR)、纤维蛋白原(FIB)水平,观察2组治疗过程中出现的不良反应.结果 治疗2周后,观察组心绞痛疗效及心电图疗效均显著优于对照组(P均＜0.05);2组炎症因子指标、血液流变学指标水平均较治疗前明显降低(P均＜0.05),且观察组降低程度显著优于对照组(P均＜0.05).2组内皮功能指标ET、TXB2水平均较治疗前明显降低(P均＜0.05),NO水平则明显升高(P均＜0.05),且观察组改善程度显著优于对照组(P均＜0.05).2组治疗过程中均未发生严重不良反应.结论 丹红注射液联合参松养心胶囊能有效改善心血瘀阻证UAP患者临床症状,抑制炎症反应,减轻血管内皮细胞损坏,共同发挥抗炎和保护血管内皮功能作用,安全有效,值得临床推广应用.%Objective It is to study the clinical curative of Danhong injection combined with Shensong-Yangxin capsule on unstable angina pectoris (UAP) of heart blood stasis syndrome and vascular endothelial function and inflammation factors in the patients.Methods 110 cases of patients with UAP with heart blood stasis syndrome were randomly divided into control group (n =55) and observation group (n =55),both groups were treated with conventional western medicine,additionally the control group was given Danhong injection by intravenous drip and observation group was given Danhong injection by intravenous drip + Shensong-Yangxin capsule orally.After treatment for 2 weeks,the curative effect of angina pectoris and electrocardiogram (ECG) were evaluated,the levels of TNF alpha,hs-CRP and interleukin-6 (IL-6),endothelin (ET),thromboxane B2 (TXB2),nitric oxide (NO),hemorheology index (WBV),HCT,ESR and fibrinogen (FIB) before and after treatment were compared in both groups.The occurrence of adverse reactions during treatment was observed in both groups.Results After two weeks of treatment,the curative effects of angina pectoris and ECG in observation group were significantly better than that in the control group (P ＜ 0.05);the levels of inflammatory factor index,hemorheology index and endothelial function index ET,TXB2 were significantly decreased (P ＜ 0.05),while NO level was increased significantly in both groups (P ＜0.05),and the improvements in observation group were better than that in control group (P ＜ 0.05).No serious adverse reactions occurred in both groups during treatment.Conclusion Danhong injection combined with Shensong-Yangxin capsule can effectively improve clinical symptoms,inhibit inflammatory reaction,relieve damage of endothelial cells in the treatment for UAP with heart blood stasis,they jointly play a role of anti-inflammatory and protecting endothelial function with good safety and effectiveness,and they are worthy of clinical popularization and application.
