乌司他丁联合大黄对急性胰腺炎患者血清淀粉酶、IL-6及TNF-α水平的影响

Objective It is to study the effect of ulinastatin combined with rhubarb on serum levels of amylase,IL-6 and TNF-α in patients with acute pancreatitis.Methods 126 patients with acute pancreatitis were randomly divided into combination group,ulinastatin group and control group.All the three groups were treated with routine therapy of acute pancreatitis.Addtionally,ulinastatin group was treated with ulinastatin,combination group was treated with ulinastatin and rhubarb.The total effective rate,symptom improving time and the changes of serum amylase,TNF-α and IL-6 before and after treatment among the three groups were compared.Results There was no significant differences in total effective rate among the three groups (P>0.05).Relief time of abdominal pain,recovery time of blood and urine amylase and the average length of hospital stay were shorter in ulinastatin group and combination group than that of control group (P<0.05).The recovery time of blood and urine amylase in combination group was shorter than that in ulinastatin group (P<0.05).The serum levels of TNF-α,IL-6 and amylase after treatment in combination group were significantly lower than that in the other two groups (P<0.05),and the levels of these indexes in ulinastatin group was lower than that in control group (P<0.05).Conclusion Ulinastatin combined combined with Dahuang could effectively inhibit cascade reaction of inflammation in patients with acute pancreatitis,relieve pancreatic injury and disease progression.%目的 研究乌司他丁联合大黄对急性胰腺炎患者血清淀粉酶、IL-6及TNF-α水平的影响.方法 将129例急性胰腺炎患者随机均分为联合组、乌司他丁组及对照组.3组均采用急性胰腺炎常规治疗方案,乌司他丁组加用乌司他丁治疗,联合组加用乌司他丁及大黄治疗.对比3组治疗总有效率、症状改善时间及治疗前后血清淀粉酶、TNF-α与IL-6.结果 3组总有效率比较差异无统计学意义(P均>0.05).联合组及乌司他丁组腹痛缓解时间、血尿淀粉酶恢复时间及住院时间均低于对照组(P均<0.05).联合组血尿淀粉酶恢复时间低于乌司他丁组(P<0.05);治疗后联合组TNF-α、IL-6及血清淀粉酶水平均低于其余2组(P均<0.05);乌司他丁组TNF-α、IL-6及血清淀粉酶水平均低于对照组(P均<0.05).结论 乌司他丁联合大黄可有效控制急性胰腺炎患者炎症瀑布级联反应,减轻胰腺损伤及病情进展.
