ABCD2评分联合躯干共济失调评分用于预测眩晕患者脑梗死的价值

         

摘要

目的 探讨 ABCD2评分联合躯干共济失调评分用于预测眩晕患者发生脑梗死的价值.方法 收集70例眩晕患者临床资料,并进行 ABCD2评分、躯干共济失调评分和影像学检查.根据最终检查结果将患者分为脑梗死组(观察组)及非脑梗死组(对照组),比较2组一般资料、ABCD2评分的差异,通过 ROC曲线评估 ABCD2评分法及躯干共济失调评分的应用价值.结果 2组性别、年龄、伴糖尿病情况比较差异均无统计学意义(P均 >0.05),观察组血压≥140/90 mmHg(1 mmHg=0.133 kPa)者明显多于对照组(P <0.05),ABCD2评分明显高于对照组(P<0.05);ABCD2评分和躯干共济失调评分越高,眩晕患者发生脑梗死的可能性越大;ROC曲线分析显示,AUC =0.76,95.0% CI [0.716, 0.698],P=0.000,Cutoff值 =4.0分,诊断灵敏度为87.50%、特异度为81.00%.结论 ABCD2评分联合躯干共济失调评分对孤立性眩晕患者发生脑梗死有一定预测价值,对脑卒中预防、预后改善均有重要意义,且该种评分特异度、灵敏度相对较高.%Objective It is to evaluate the value of ABCD2 score combined with trunk ataxia score for predicting cere-bral infarction in the patients with vertigo. Methods Clinical data of seventy patients with vertigo were collected,and AB-CD2 scores,trunk ataxia scores,and imaging examinations were performed. According to the final test results,the patients were divided into cerebral infarction group (observation group)and non-cerebral infarction group (control group). The general data and ABCD2 score of the two groups were compared. The application value of ABCD2 score method and trunk ataxia score were evaluated by ROC curve. Results There was no significant difference in gender,age and concomitant dia-betes between the two groups (P>0.05). The number of patients with blood pressure≥140/90 mmHg was significantly more while the ABCD2 score was significantly higher in the observation group than that in the control group (P <0.05). The higher the ABCD2 score and trunk ataxia score,the greater the likelihood of cerebral infarction in patients with vertigo;ROC curve analysis showed that AUC=0.76,95.0% CI [0.716,0.698],P=0.000,Cutoff value =4.0 points,the di-agnostic sensitivity was 87.50%,and the specificity was 81.00%. Conclusion ABCD2 score combined with trunk ataxia score has certain predictive value for cerebral infarction in the patients with isolated vertigo,and it is of great significance for prevention of stroke and improvement of prognosis. The score specificity and sensitivity are relatively higher.
