弥散张量成像在缺血性脑白质疏松症中的应用初步研究

     

摘要

目的:探讨磁共振弥散张量成像(DTI)在脑白质疏松症(LA)认知功能障碍的应用价值。方法对15例腔隙性脑梗死伴发脑白质疏松症患者(LA组)及8例单纯腔隙性脑梗死患者(LI组)和8例正常老年人(对照组)行常规MRI及DTI检查。测定兴趣区分数各向异性(FA)值和表观弥散系数(ADC)值,并对与蒙特利尔认知评估量表(MoCA)评估结果进行相关性分析。结果 LA组与LI组、正常对照组比较,FA值降低,ADC值升高,差异均具有显著性,兴趣区是双侧额叶白质、右侧海马、胼胝体膝部及压部(P<0.05)。各部位FA值与MoCA评估得分呈正相关。结论 LA出现认知功能障碍部位以双侧额叶白质、胼胝体受损为主,DTI可以发现常规MRI检查所不能显示的脑白质的超微结构改变。%Objective To discuss the application value of diffusion tensor imaging(DTI) in leukoaraiosis (LA) cognitive dysfunction. Methods 15 patients of lacunar infarction with concomitant leukoaraio-sis (LA group), 8 patients of isolated lacunar infarction alone (LI group) and 8 cases of normal elderly (normal control group) were examined with routine MRI (magnetic resonance imaging, MRI) and DTI scans. AOI fractional anisotropy (FA) values and the apparent diffusion coefficient (ADC) value were determined, and assessment results of Montreal Cognitive Assessment(MoCA) were cor-related analyzed. Results Comparison of LA group and LI group, normal group and control group, FA values were significantly reduced and ADC values were significantly increased. The differences were statistically significan. Area of interest is the bilateral frontal white matter, right hippocampus, corpus callosum and the press section (P<0.05). Various parts of the FA values and MoCA assessment scores were positively correlated. Conclusion LA cognitive dysfunction parts was mainly with the damages of bilateral frontal white matter and corpus callosum. The technique of DTI can be found ultrastructural changes which can not be displayed by conventional MRI.
