CT定量测量与慢性阻塞性肺疾病患者肺功能相关性分析

摘要

目的：探讨慢性阻塞性肺疾病(COPD)患者 MSCT 定量测量肺密度及肺容积指标与肺功能指标的关系。方法选取2014年7月~2015年7月60例 COPD 患者为 COPD 组，60例健康志愿者为对照组，均行胸部 MSCT 检查及肺功能检查检查。测量并比较 COPD 组和对照组 MSCT 定量测量指标及肺功能指标的差异，观察 COPD 患者 MSCT 定量测量指标与肺功能指标的相关性。结果COPD 组 Vex、Vin、Vex/Vin 均显著高于对照组，MLDex、MLDin、Vin-Vex 及 MLDex-MLDin 均显著低于对照组，差异具有统计学意义(P<0.05)，COPD 组 FEV1、FVC 及FEV1/FVC 均显著高于对照组，RV、TLC、RV/TLC 均显著低于对照组，差异具有统计学意义(P<0.05)，经相关性分析显示，MLDex-MLDin 与肺功能指标 FEV1、FVC 及 FEV1/FVC 呈正相关，RV/TLC 呈负相关(P<0.05)。Vex/Vin 与肺功能指标 RV/TLC 存在正相关，Vin-Vex 与 FVC 具有正相关(P<0.05)。结论COPD 患者 MSCT 定量测量肺密度及肺容积指标均出现明显的改变，并与临床肺功能检测具有较好的一致性。%Objective To investigate the chronic obstructive pulmonary disease (COPD) patients with MSCT quantitative measurement of lung density and lung volume index and lung function index. Methods 60 cases of COPD patients from July 2014 to July 2015 were chosen as COPD group,60 healthy volunteers as control group,Underwent chest MSCT examination and pulmonary function examination. Measured and compared group COPD and group MSCT quantitative measurement indexes and pulmonary function index control,The correlation observed in patients with COPD MSCT quantitative measurement index and lung function index.Results The Vex,Vin,Vex/Vin of COPD group were significantly higher than control group,MLDex,MLDin,Vin-Vex and MLDex-MLDin were significantly lower than the control group,The difference was statistically significant (P<0.05),FEV1,FVC and FEV1/FVC in COPD group were significantly higher than those in control group,RV,TLC and RV/TLC were significantly lower than the control group,The difference was statistically significant (P<0.05),The correlation analysis showed that,correlation between MLDex-MLDin and lung function index of FEV1,FVC and FEV1/FVC was positive,and was negatively related to RV/TLC(P<0.05).There was a positive correlation of RV/TLC Vex/Vin and lung function index,Vin-Vex and FVC had positive correlation(P<0.05).Conclusion MSCT quantitative measurement of lung density and lung volume indexes in patients with COPD change significantly,they have good consistency with the clinical pulmonary function test.
