目的：：比较局部姑息性放疗与口服阿片类药物治疗肿瘤骨转移患者疼痛的临床疗效。方法：选取200例肿瘤骨转移患者作为研究对象，随机分为对照组和观察组，每组各100例。对照组患者使用短效阿片类药物滴定后改成长效缓释阿片类药物口服镇痛治疗；观察组患者进行6MV-X线直线加速器调强放射治疗。结果：治疗后，观察组患者的疼痛完全缓解8例，部分缓解86例，无效6例，总有效率为94%；对照组患者的疼痛完全缓解7例，部分缓解85例，无效8例，总有效率为92%；两组患者临床治疗有效率比较无统计学意义(P>0.05)；观察组患者不良反应发生率低于对照组(P<0.05)，具有统计学意义。结论：肿瘤骨转移疼痛患者采用局部姑息性放射治疗和口服阿片类药物均具有镇痛的效果，但局部姑息性放射治疗不良反应少，可改善患者生活质量。%Objective:To compare clinical effects of local palliative radiotherapy and oral Opioids in treatment of pain of bone metastasis tumor. Methods:200 patients who suffered from the pain of bone metastasis tumor were included as the research objects, and these patients were randomly divided into control group and observation group, 100 cases in each group. The control group was ti-trated with the short acting opioids firstly, and then orally took long-acting and sustained-release tablets, while the observation group accepted the 6MV- X-ray linear accelerator intensity modulated radiation therapy. The clinical therapeutic effects of all the bone me-tastasis cancer patients with pain were observed and analyzed. Results: ( 1 ) After the treatment, in the observation group, 8 cases were completely remitted, 86 cases were partly remitted, 6 cases were invalid, and the total effective rate was 94%, while in the con-trol group, 7 cases were completely remitted, 85 cases were partly remitted, 8 cases were invalid, and the total effective rate was 92%. The difference of the clinical effective rate between the two groups was not statistically significant (P>0. 05). The rate of ad-verse reactions in observation group was lower than that in control group, and the difference was statistically significant (P<0. 05). Conclusions:In the patients with bone metastasis pain, the local palliative radiotherapy and oral opioid drugs can achieve good analge-sic effect, but the local palliative radiation therapy has less adverse reactions and can improve the patients' quality of life.
