仪仗兵训练性血红蛋白尿发病机制及其影响因素探讨

     

摘要

目的:通过观察仪仗兵血红细胞抗氧化能力相关指标的变化,探讨训练性血红蛋白尿可能的发生机制及影响因素。方法观察70名仪仗兵一次常规训练前后血红细胞丙二醛( malondialdehyde,MDA)、超氧化物歧化酶( superoxide dis-mutase,SOD)及血浆总抗氧化能力( total antioxidant capacity,AOC),血乳酸( blood lactic acid,BLA)变化,再以是否反复出现肉眼血尿分组,观察上述指标的变化及与肉眼血尿的相关性。结果训练前MDA(6.57±0.64)nmol/L,训练后(6.86±0.67) nmol/L;训练前SOD(9.61±1.08) U/mg,训练后(10.46±2.12) U/mg;训练前T-AOC 5.18(3.70~6.11) U/ml,训练后7.89(7.09~9.81)U/ml;训练前BLA(21.4±5.4)mmol/L,训练后(31.8±12.8)mmol/L。 MDA、SOD、T-AOC、BLA训练后均较训练前显著升高(P<0.05)。 MDA、SOD、T-AOC训练前后的差值与血尿的出现具有显著的相关性(P<0.05)。结论仪仗兵常规训练强度可引起红细胞抗氧化能力变化,其差异可能是训练性血红蛋白尿发生原因之一。%Objective To study the possibility of honor guard training-induced hemoglobinuria, occurring mechanism and in-fluencing factors.Methods To observe the soldiers’ blood erythrocyte malondialdehyde ( MDA) , superoxide dismutase ( SOD) , and plasma total antioxidant capacity ( T-AOC) , blood lactate ( BLA) changes before and after a routine training in the honor guard, and divide them into two groups with whether they developed naked eye“hematuria” repeatly in order to observe the change in the index of and correlation with“the naked eye hematuria”.Results The MDA before training(6.57 ±0.64)nmol/mg, after training (6.86 ± 0.67) nmol/mg; SOD before training (9.61 ±1.08) U/mg, after training (10.46 ±2.12 ) U/mg; T-AOC before training 5.18 (3.70-6.11) U/ml, after training (7.09-9.81) U/ml; BLA before training (21.4 ±5.4) mmol/L, after training (31.8 ±12.8) mmol/L.MDA, SOD, T-AOC, BLA after training increased compared with those before training increased, with statistical significance (P<0.05).Difference in MDA, SOD, T-AOC (P<0.05) existed with correlation hematuria before and after training.Conclusions The honor guards’ antioxidant capacity of red blood cell, is changed by usual training; it may be one of the causes of training-in-duced hemoglobinuria.
