CD4+细胞计数在艾滋病患者机会性感染护理指导中的应用研究

摘要

目的：探讨艾滋病患者机会性感染护理干预中应用CD4+细胞计数指导的临床效果。方法：临床纳入113例本院2014年3月-2015年9月收治的艾滋病患者，根据临床护理方式的不同分为两组。其中53例患者采用常规临床护理作为对照组，另60例患者采用以CD4+细胞计数为指导的临床护理干预措施。观察两组患者护理后的临床效果情况，如感染率、病死率及生命系数等。结果：两组患者护理后，病情均有所好转，观察组感染率为15.0%，明显低于对照组的45.3%，观察组病死率为6.7%，明显低于对照组的22.6%，观察组患者生命系数中疼痛、精力、睡眠状态、躯体活动及情绪反应方面均明显优于对照组，差异均有统计学意义（P<0.05）；而社交隔离方面两组相比较，差异无统计学意义（P>0.05）；本次研究中，CD4+细胞计数水平以≤50个/μL的感染率和死亡率较高，其次是CD4+细胞计数水平为51~100个/μL，而感染率和死亡率较低的为CD4+细胞计数水平为101~200个/μL，观察组明显低于对照组，两组比较差异均有统计学意义（P<0.05）。结论：CD4+细胞计数指导临床艾滋病患者的护理干预，有助于护理人员观察患者的病情变化，有效的预测疾病转归和可能发生的机会性感染，及时修正并制定针对性的护理措施，CD4+细胞计数指导的理论依据确切，提高了临床护理效果和治疗效果，值得临床应用及推广。%Objective:To investigate the clinical effect of CD4+ cell count in application of opportunistic infections nursing instruction in patients with AIDS.Method:A total of 113 cases of AIDS patients admitted to our hospital from March 2014 to September 2015 were divided into two groups according to the different clinical nursing methods.Among them,53 patients were treated with routine clinical nursing as the control group,and the other 60 patients were treated with CD4+ cell count as the guiding clinical nursing intervention as the observation group. The clinical effect of two groups after care,such as the patient’s infection rate,mortality and life coefficient,etc were Observed.Result:After nursing care in two groups,the illness had been better,but the infection rate in the observation group was 15.0%,was significantly lower than 45.3% of the control group,pain,energy,sleep, physical activity and emotional reaction of the patients in the observation group were significantly better than those of the control group,the results were statistically significant (P<0.05).The social isolation of two groups,the difference was not statistically significant(P>0.05).In this study,CD4+ cell count was less than or equal to 50/μL infection rate and mortality rate were higher,followed by CD4+ cell count for 51-100/μL,and infection rate and mortality rate were low CD4+ cell count for 101-200/μL,the observation group was significantly lower than that of the control group,the result was statistically difference(P<0.05).Conclusion:CD4+ cell count guidancing nursing intervention in patients with AIDS,is help to the nursing staff to observe the patient's condition changes,effectively predict disease prognosis and possible opportunistic infection,update and develop for nursing measures,CD4+ cell count under the guidance of the theoretical basis for the exact,improves the effect of clinical nursing and treatment effect,it is worthy of clinical application and promotion.
