无抽搐电休克与重复经颅磁刺激治疗精神分裂症患者顽固性幻听的临床对照研究

摘要

目的：研究无抽搐电休克（ MECT）与重复经颅磁刺激（ rTMS）治疗精神分裂症顽固性幻听的临床疗效及安全性，探索精神分裂症顽固性幻听临床治疗方法。方法将顽固性幻听患者随机分为MECT组（50例）和rTMS组（50例）。 MECT组行MECT治疗6次，rTMS组则行rTMS治疗10次，治疗期间维持原抗精神病药物剂量及种类不变。采用PANSS、TESS等量表进行临床疗效评定及不良反应分析。结果治疗后，MECT组与rTMS组的有效率比较差异无统计学意义（ P＞0．05）。两组在治疗后与治疗前比较，PANSS评分均显著下降，差异有统计学意义（P＜0．05）。两组整体有效率比较差异无统计学意义（P＞0．05）。 MECT组发生不良反应例数明显高于rTMS组，两组比较差异有统计学意义（P＜0．05）。结论 MECT及rTMS治疗精神分裂症顽固性幻听疗效确切，rTMS安全性好。%Objective To study auditory hallucination clinical curative methods, the clinical curative effect and safety on schizophrenia and auditory hallucination by using Electric Convulsive Treatment and Repetitive Transcranial magnetic stimulation.Methods Split the auditory hallucination patients into two groups: MECT group (5-0 cases) and rTMS group (50 cases).The MECT group took MECT treatment for 6 times, the rTMS group takes rTMS treatment 10 times, during the treatment kept the old anti-schizophrenia drug dose and species. Then taking PANSS, TESS meters to run clinical curative effect evaluation and compared their efficiency. Results After the treatment, the differences between MECT group and rTMS group had no statistics meanings (P>0.05).By comparing the time before and after treatment of the two groups, all scores had decreasedsignificantly.Their difference then had the statistics meanings (P<0.05).The effective comparing difference between tow groups had no statistics meaning ( P>0.05).The number of cases of the bad body response in MECT group was much larger than that of rTMS group.The comparing difference between two group had statistics meaning (P<0.05).Conclusions The MECT and rTMS treatment has the effect on healing schizophrenia auditory hallucination.rTMS is better in safety than others.
