重庆市二级及以上医院静脉留置针应用现状及影响因素分析

摘要

目的:了解重庆市二级及以上医院临床护士应用外周静脉留置针的现状及影响因素.方法:采用自行设计的问卷对重庆市9所二级、三级医院共958名临床护士进行调查和分析.结果:静脉留置针总体使用率为59.31%,其中二级医院平均使用率为51.31%,三级医院平均使用率为63.30%,护士执行静脉留置针封管浓度正确率为42.3%,留置时间正确率为43.3%,无菌透明敷料更换时间的正确率仅为6.8%;二、三级医院静脉留置针规范使用率比较差异有统计学意义(P<0.01),儿科护士优于其他科室护士(P<0.01);75%以上的护士认为费用高和患者感觉不舒适是影响静脉留置针使用率的主要因素,65%的护士认为工作太忙是导致静脉留置针使用不规范的主要原因.结论:静脉留置针使用及维护中仍存在问题,需进一步加强护士的培训和质控力度,从而提高静脉留置针使用率,切实保障患者输液安全.%Objective:To investigate the status and influencing factors of peripheral venous indwelling needle for clinical nurses in hospitals of grade two and above in Chongqing.Methods:A total of 958 clinical nurses from 9 hospitals of grade two or grade three in Chongqing were investigated and analyzed by self-designed questionnaire.Results:Venous indwelling needle overall utilization rate was 59.31%,of which grade two hospital average utilization rate was 51.31%,of which grade hospital average utilization rate was 63.30%.The correct rate of implementation of venous indwelling needle with concentration was 42.3%,and the correct indwelling time rate was 43.3%.The correct rate of sterile transparent dressing change time was only 6.8%.There was significant difference in the standard utilization rate of venous indwelling needle between the grade two or grade three hospitals(P<0.01).Pediatric nurses were better than other nurses(P<0.01).More than 75% of the nurses thought that the high cost and feel uncomfortable were the main factors affecting the use rate of venous indwelling needle,and 65% nurses thought that too much work was the main reason leading to the incorrect use of venous indwelling needle.Conclusion:There are still some problems in the use and maintenance of venous indwelling needle.It is necessary to further strengthen the training and quality control of nurses,so as to improve the utilization rate of intravenous indwelling needle and ensure the safety of patients infusion.
