磁共振张量成像基底节区脑出血致远端锥体束继发损害的研究

     

摘要

Objective To study the changes of fractional anisotropy( FA) in patients with cerebral hemorrhage of basal ganglia using MR diffusion tensor imaging (DTI). Methods 30 cases of acute cerebral hemorrhage of basal nuclei area underwent DTI. FA variations on the region of interest(ROI) of pyramidal tract in the bleeding affected side, unaffected side of cerebral peduncles and pons were measured and compared. Results The FA value of affected and unaffected sides in the 30 cases of acute cerebral hemorrhage in basal nuclei area showed statistical differences in cerebral peduncles (0. 50 ± 0. 05 ) (t = 5. 15,P<0. 05), but no statistical difference in pons( 0. 4 5 ± 0. 05 vs 0. 47 ± 0. 05 ;t= 1. 4 ,P>0.05 ). Conclusion DTI not only can show the changes of pyramidal tract of acute cerebral hemorrhage in basal nuclei area, but also can assess the injury extent by measuring FA value.%目的 研究磁共振弥散张量成像(diffusion tensor imaging,DTI)对基底节区急性脑出血病例锥体束的各向异性值(fractional anisotropy,FA)的变化.方法 收集30例基底节区急性脑出血病例,用弥散张量成像方法,测量出血灶同侧、对侧位于大脑脚和桥脑平面感兴趣区(region of interest,ROI)内的锥体束FA值变化,比较病灶同侧和对侧相同区域范围内FA值差异.结果 30例基底节区急性脑出血病例的病灶侧大脑脚平面ROI区锥体束FA值(0.45±0.06)明显低于对侧ROI区锥体束FA值(0.50±0.05)(t=5.15,P<0.05),桥脑平面ROI区域的FA值略低于对侧但差别无统计学意义(0.45±0.05 vs 0.47±0.05;t=1.4,P>0.05).结论 DTI不仅可直观显示基底节区急性脑出血纤维束的受损改变,还可通过测量FA值的变化量化评估基底节区急性脑出血锥体束损伤的程度.
