微型锁定钢板与普通钢板内固定在掌指骨骨干骨折中的疗效比较

         

摘要

目的 观察掌指骨骨干骨折采用微型锁定钢板与普通钢板内固定术的临床疗效.方法 2015年8月-2016年12月,对62例掌指骨骨干骨折患者按随机数字表法分为观察组和对照组,各31例.观察组采用微型锁定钢板内固定术,对照组采用普通钢板内固定术,比较两组患者手术时间、临床愈合时间及并发症发生率,并采用中华医学会手外科学分会制定的上肢部分功能评定标准对手部功能进行评分.结果 两组患者在手术时间、骨折愈合时间方面比较差异无统计学意义(P>0.05);术后3个月,观察组手部功能评分优良率为93.55%(29/31),对照组手部功能评分优良率为74.19%(23/31),比较差异具有统计学意义(χ2=4.292,P<0.05);观察组术后并发症发生率为3.22%(1/31),对照组并发症发生率为19.35%(6/31),比较差异具有统计学意义(χ2=4.026,P<0.05).结论 微型锁定钢板相比于普通钢板内固定在治疗掌指骨骨干骨折中疗效显著,手部功能恢复良好,并发症少,具有较高的临床价值,但由于价格昂贵,在简单掌指骨骨干骨折中仍不作为常规推荐使用.%Objective To observe the clinical curative effect of mini-locking plate and ordinary plate internal fixation on metacarpal and phalangeal shaft fracture. Methods Sixty-two patients with metacarpal and phalangeal fractures were selected from August 2015 to December 2016. According to the random number table method, 31 cases were divided into observation group and control group. The observation group was treated with miniature locking plate internal fixation The operation time, clinical healing time and the incidence of complications were compared between the two groups. The function of the upper part of the upper limb was evaluated by the Chinese Medical Association Hand Surgery Branch. Results There was no significant difference in operative time and fracture healing time between the two groups (P> 0.05). The excellent rate of hand function was 93.55% (29/31) at 3 months after operation, and the excellent rate of hand function score was 74.19% (23/31) in the control group, the difference was statistically significant (χ 2=4.292, P<0.05);The incidence of postoperative complications was 3.22% (1/31) in the observation group and 19.35%(6/31) in the control group, the difference was statistically significant (χ 2=4.026, P<0.05). Conclusion Compared with ordinary plate fixation, mini-locking plate is effective in the treatment of metacarpal and phalangeal fractures, and the recovery of hand function is good and the complication is low. It has high clinical value, but it is still expensive in simple bone fracture not recommended as a regular therapy.
