目的：探讨应用三位一体化护理模式（即医院‐社区‐家庭护理模式）用于慢性阻塞性肺疾病（COPD）患者出院后肺康复的效果。方法将80例COPD患者按住院号随机分为干预组和对照组，两组患者出院后均给予常规护理，干预组依据医院‐社区‐家庭护理模式制定康复计划并实施，两组患者在干预前、干预后1年进行肺功能测定和生活质量评分。结果患者干预后1年的肺功能监测指标比较，干预组与对照组比较有明显改善，差异有统计学意义（P＜0．05）。干预组患者干预后1年生活质量得分与对照组比较，差异有统计学意义（P＜0．05）。结论以三位一体化护理模式对COPD患者进行肺康复管理，有利于改善患者的肺功能，提高生活质量。%Objective To study the effect of hospital‐community‐family‐care for pulmonary rehabilitation among patients with chronic obstructive pulmonary disease (COPD) .Method Totally 80 patients were randomly divided into experimental group and control group ,40 cases in each group .Patients in experimental group received the pulmonary rehabilitation based on hospital‐community‐family‐care ,while patients in control group received routine care .All pa‐tients were investigated with airways questionnaire 20 (AQ20‐R) and pulmonary function index before and after in‐tervention .Result The pulmonary function were significantly improved in the experimental group compared with that in the control group(P<0 .05) .The scores of AQ20‐R for patients in experimental group were lower than that the control group after admission(P<0 .05) .Conclusion The pulmonary rehabilitation program based on hospital‐com‐munity‐family‐care can improve the quality of life and lung function index for patients with COPD .
