康复护理延伸训练对步行痉挛型脑瘫患儿三维步态时空和表面肌电的影响

         

摘要

Objective To observe effect of continuous rehabilitation nursing training on three dimensional gait,spatiotemporal and surface electromyography(sEMG) for children with spastic cerebral palsy.Methods 63 patients were randomly divided into the contros group and the observation group,The patients in control groap was given convential rehabilitation training,patients in observation groap was given rehabilitation nursing continuing training and conventional rehabition training.after 2 courses of treatment,the patients' foot three-dimensional gait and iEMG、AEMG、CR before and after the treatment of the left leg tibialis anterior muscle and gastrocnemius measured and compared.Results The three-dimensional gait parameters,iEMG of the gastrocnemius in the passive state,the iEMG、MF、CR of the gastrocnemius and tibialis anterior muscle in the active state for the two groups after treatment were better improved than that before treatment,There was significant difference for two groups (P<0.05 or P< 0.01).The above indicators of group B were better improved than that of group A,there was significant difference between the two groups(P<0.05).Conclusion Based on the conventional rehabilitation training,the rehabilitation nursing continuing training could improved the indicators of foot three-dimensional gait and the methods of electricity for children with walking spastic type cerebral palsy,which worthy of clinical promotion.%目的 观察康复护理延伸训练对步行痉挛型脑瘫患儿三维步态时空和表面肌电等指标的影响.方法 将63例步行痉挛型脑瘫患儿随机分为对照组31例和观察组32例;对照组予以常规康复训练治疗,观察组予以康复护理延伸训练加常规康复训练治疗.两组分别治疗2个疗程,检测治疗前、后三维步态时空参数和左下肢腓肠肌和胫骨前肌的iEMG、AEMG、CR等指标.结果 两组患儿治疗后的三维步态参数,胫骨前肌和腓肠肌屈伸最大等长收缩状态下的iEMG、MF、CR,腓肠肌被动活动状态下的iEMG,与治疗前相比,均有不同程度的改善,差异具有统计学意义(P<0.05或P<0.01);治疗后观察组与对照组比较,上述指标差异均具有统计学意义(P<0.05).结论 在常规康复训练的基础上,康复护理延伸训练能更好地改善步行痉挛型脑瘫患儿三维步态时空和表面肌电等各参数,值得临床推广.
