'入院-出院-家庭随访'一站式健康宣教中心的建立与实践研究

         

摘要

目的 探讨"入院-出院-家庭随访"一站式健康宣教中心在住院患者中的实施效果.方法 选择2015年1-12月入住我院骨科患者520例,随机分为观察组、对照组各260例.对照组采用常规出入院宣教模式进行管理;观察组通过"一站式健康宣教中心"模式进行管理.比较两组模式实施后患者平均出、入院手续办理时间、护理人力工时数及护患对两组模式的满意度.结果 观察组平均入、出院手续办理时间明显少于对照组,差异有统计学意义(P<0.05);观察组护理工时数低于对照组,差异有统计学意义(P<0.05);观察组患者满意度(99.6%)高于对照组(92%),差异有统计学意义(P<0.05).结论 "入院-出院-家庭随访"一站式健康宣教中心实现了患者从入院到出院全程服务项目的有效整合,优化了服务流程,延伸了护理服务内涵,提升了患者满意度,符合患者利益.中心的成立同时也减少了护士工作量,一定程度地缓解了病房护士人力资源紧张的现状,符合医院的需要.%Objective To explore the implantation effect of the one stop health education center of "admission-discharge and family follow-up" for hospitalized patients.Methods 520 cases of hospitalized patients in department of orthopedics in Shanghai sixth people's hospital from December 2016 to January 2015 were selected and randomly divided into experimental group and control group, 260 cases in each group.Patients in control group was given routine mode of health education.Patients in experimental group was managed by the "one stop health education center" mode.The average time of admission procedure and discharge formalities, the number of nursing hours and the satisfaction degree of patients to nursing for both groups were compared after the implementation of this project.Results The average admission and discharge procedures in experimental group were significantly less than that of the control group (P<0.05).The nursing hours in experimental group were less than that in the control group (P<0.05).The patient satisfaction degree (99.6%) in experimental group was significantly higher than that (92%) of the control group (P<0.05).Conclusion One-stop health education center of "admission-discharge and family follow-up" fulfills to reach the effective integration of the whole service for patients from admission to discharge, service process was optimized, and extends the connotation of nursing services, furthermore, improves patient satisfaction and interests of patients.The establishment of the center reduces the workload of nurses to a certain extent eased the current situation of nurses of human resources and then meets the hospital interests.
