深度水解蛋白配方奶联合酪酸梭菌对早产儿早期喂养后血生化的影响

         

摘要

目的:探讨深度水解蛋白配方奶(extensively protein hydrolyzed formula,eHPF)联合酪酸梭菌对早产儿早期喂养后血生化的影响。方法:将2014年7月1日~2016年3月31日,生后24h内入住本院新生儿科的早产儿(28周≤胎龄≤34周且1000g≤出生体重≤2000g)90例,随机分为实验组(eHPF联合酪酸梭菌组)、对照组①(eHPF组)及对照组②(早产儿配方奶Premature infant for-mula,PTF组),各组30例,观察三组早产儿生后d1、d7、d14的血生化指标(总蛋白T-Prot、白蛋白ALB、谷丙转氨酶ALT、总胆红素TBil、间接胆红素TB-DB、肌酐Cr、尿素氮BUN、乳酸脱氢酶LDH)及每日经皮胆红素值的变化,并进行统计分析。结果:生后d1各项血生化指标无明显差异(P>0.05);生后d7的总胆红素值及间接胆红素值实验组较两组对照组低,差异有统计学意义(P<0.05),其他血生化指标差异无统计学意义(P>0.05);生后d14的总蛋白及白蛋白实验组较两组对照组高,差异有统计学意义(P<0.05),其他血生化指标差异无统计学意义(P>0.05)。三组病例血清最高TBIL值实验组较两组对照组低,P<0.05,差异有统计学意义,血清TBIL值降至正常所需时间实验组较两组对照组短,P<0.05,差异有统计学意义。结论:eHPF联合酪酸梭菌对早产儿早期喂养,有利于早产儿胆红素代谢,减轻胆汁淤积,提高血清总蛋白及白蛋白水平。%Objective:To observe the influence of blood biochemistry of extensively hydrolyzed protein formula (eHPF) combined with clostridium butyricum on early feeding of premature infants. Methods:90 premature infants(28 weeks≤gestational age≤34 weeks and 1000g≤birth weight ≤2000g)admitted in neonate department in our hospital from July 1, 2014 to March 31, 2016. They were assigned into 3 group randomly: treatment group (eHPF combined with clostridium butyricum),control group ①(eHPF) and control group②(PTF). Each group 30 cases.Observe the blood biochemical indicators(T- Prot, ALB, ALT, TBil,TB–DB, Cr, BUN,and LDH) and daily transcutaneous bilirubin value of three groups on Day 1,Day 7,Day 14,and analyze. Results:The blood biochemical indicators have no significant statistical difference (P>0.05) on Day1;the TBil and TB –DB level is significantly lower in treatment group than control group ①and ② on Day 7, P<0.05; the T - Prot, and ALB level is significantly higher in treatment group than control group ①and②on Day 14, P<0.05.The highest TBIL is lower in treatment group than control group ①and ②,P<0.05;the time of the TBIL to normal is shorter in treatment group than control group ①and ②,P<0.05.Conclusion: eHPF combined with clostridium butyricum on early feeding of premature infants can help the premature bilirubin metabolism, reduce cholestasis,and raise the level of serum total protein and albumin.
