目的:研究1 470 nm激光前列腺剜除术治疗前列腺增生症的近期临床疗效及安全性.方法:小结2015年12月~2016年3月采用国产1 470 nm激光行前列腺剜除术的28例患者临床资料.结果:28例患者手术均顺利完成,平均手术时间(94.0±19.7)min,术中出血少,无输血患者.术前与术后即刻钠离子浓度差异无统计学意义(P>0.05).术后膀胱冲洗时间(1.6±0.5)d,留置导尿管时间(4.1±1.0)d,住院时间(6.0±1.5)d.术后3个月随访,国际前列腺症状评分、生活质量评分、最大尿流率及剩余尿量均较术前明显改善,且差异有统计学意义(P<0.01).4例出现暂时性尿失禁,经盆底肌锻炼均在1个月内恢复;无尿道狭窄及需二次手术患者.结论:1 470 nm激光前列腺剜除术治疗前列腺增生症是一种安全、有效,且并发症少,术后恢复快的手术方式.%Objective: To study the short-term clinical efficacy and safety of 1 470 nm-laser enucleation of the prostate for benign prostatic hyperplasia.Methods: The clinical data of 28 cases undergoing enucleation of prostate with domestic 1 470 nm-laser from December 2015 to March 2016 were briefly summarized.Results: The 1 470 nm-laser enucleation of the prostate was successfully performed on 28 cases,with less blood loss and no blood transfusion.The average operation time was (94.0±19.7) min.There was no significant difference in instant concentrations before and after operation (P>0.05).The postoperative bladder washing time was (1.6±0.5) days,indwelling catheter time was (4.1±1.0) days,and the length of hospital stay was (6.0±1.5) days.After a 3-month follow-up,International Prostatic Symptom score (IPSS),quality of life (QOL) score,maximum flow rates (Qmax),and postvoid residual volume (PVR) were significantly improved,and the difference was statistically significant (P<0.01).Transient urinary incontinence recovered within one month after pelvic muscle exercises in 4 cases.There were no patients with urethral stricture or the need for a further surgery.Conclusions: Treatment of benign prostatic hyperplasia with 1 470 nm-laser enucleation of the prostate is a safe and effective surgical method,with few complications and speedy recovery.
