Objective To analyze retrospectively whether the expression of topoisurnerase Ⅱα protein influences the response to anthracycline - containing first - line regimens in advanced breast cancer. Methods In this study topoisomerase Ⅱα protein was evaluated by immunohistochemistry in formalin - fixed paraffin - emhedded tissue from 200 samples of primary breast cancer. The clinical pathological parameters and chemotherapy response were evaluated. Results Over - expression of topoisomcrase Ⅱα was associated with ER, Her -2, histological grading and distant metastasis. The over - expression of topoisomerase Ⅱ - alpha ( Topo Ⅱα) in the tumors predicted response to anthracycline - containing first - line regimens. However, the expression of topoisomerase Ⅱ - alpha ( Topo Ⅱα) was not correlated with clinical response rate of the non - anthracycline - containing regimens. Conclusion The expression of topoisomerase Ⅱ - alpha in primary breast cancer tissue is a predictive factor for anthracyrline response in advanred breast cancer.%目的 观察晚期乳腺癌患者肿瘤组织中拓扑异构酶Ⅱα(topoisomerase Ⅱα,Topo Ⅱα)的表达与蒽环作为一线化疗药物疗效的关系.方法 采用免疫组化法检测200例乳腺癌石蜡切片标本中Topo Ⅱα的表达,观察其与晚期乳腺癌患者临床病理参数及近期化疗有效率之间的关系.结果 200例晚期乳腺癌远处转移部位数、组织学分级、ER和Her-2表达与Topo Ⅱα表达有关.对于应用含有蒽环方案有效的患者,其肿瘤组织Topo Ⅱα阳性表达明显高于无效者(P＜0.001).采用非蒽环类方案的患者,其肿瘤组织Topo Ⅱα表达与疗效无关.结论 Topo Ⅱα的表达状态与晚期乳腺癌患者一线化疗疗效密切相关,化疗方案中含蒽环类药物者肿瘤组织表达Topo Ⅱα近期有效率更高.
