三维T2*加权血管成像检测大脑中动脉磁敏感征的应用

             

摘要

Objective To evaluate the application value of 3D T2 star weighted angiography in detection of middle cerebral artery susceptibility sign of ischemic stroke patients. Methods Forty-eight patients with ischemic stroke were enrolled in this study. MRI features of MCA susceptibility sign on 3D SWAN were observed, and then corresponding relationship with MCA stenosis/occlusion sign on 3D MRA and the relevance ratio of middle cerebral artery susceptibility of ischemic stroke patients in different period were compared. Results The MCA susceptibility sign on 3D SWAN was revealed in continues level of MCA, whose hypointensity was wider than the contralateral artery. There was a good relevance between MCA susceptibility sign on 3D SWAN and the MCA stenosis/occlusion sign on 3D MRA (Pearson C = 0. 740. P <0. 05). The relevance ration of MCA susceptibility sign in patients with hyperacute ischemic stroke was 70%, much higher than those non-acute ischemic strokes of 28%. Differences between them were statistically significant by x2 a-nalysis with P <0. 05. Conclusion Presenting MCA susceptibility sign by 3D SWAN provides valuable information of a-cute MCA thrombotic occlusion in ischemic infarction.%目的 探讨三维T2*加权血管成像(3D SWAN)检测缺血性脑卒中患者大脑中动脉磁敏感征的应用价值.方法 回顾性分析48例大脑中动脉供血区域急性缺血性脑卒中患者MR检查的影像资料,对照分析3D TOF MRA和3D SWAN序列检测大脑中动脉磁敏感征的对应关系.并比较不同发病时间缺血性脑卒中患者大脑中动脉磁敏感征的检出率.结果 大脑中动脉磁敏感征在3D SWAN序列上表现为MCA走行区域的多个连续层面上,宽度大于对侧血管管径的条状低信号;经x2检验关联性分析,3D SWAN和3D TOF MRA对显示大脑中动脉磁敏感征的对应关系有统计学意义(P<0.05),Pearson关联系数C=0.740,显示两者间关联性较高;3D SWAN序列对超急性期脑梗塞患者大脑中动脉磁敏感征检出率(70%)高于非超急性期(28%),经x2检验,二者之间差异有统计学意义(P<0.05).结论 3DSWAN序列检测大脑中动脉磁敏感征可以提供缺血性脑卒中的血栓信息.
