益气化浊利水方对慢性左心功能不全患者心率变异性及血清NT-ProBNP影响的相关性研究

摘要

目的:研究益气化浊利水方治疗慢性左心功能不全患者心率变异性及血清氨基末端脑钠肽前体(NT-ProBNP)的变化.方法:将符合入选标准的慢性左心功能不全患者随机分为治疗组和对照组.对照组服用利尿剂、硝酸酯类药物及血管紧张素转换酶抑制剂,治疗组在西药常规治疗的基础上,加服益气化浊利水方,2组患者均在治疗前和治疗4周后通过动态心电图检测心率变异性(HRV),具体指标为SDNN、SDANN、RMSSD、PNN50,并抽血检测NT-ProBNP.结果:治疗后2组NT-ProBNP水平较治疗前降低(P＜0.05),SDNN、SDANN、RMSSD、PNN50较治疗前升高,且治疗组较对照组治疗后NT-ProBNP水平明显降低,SDNN、SDANN、RMSSD、PNN50明显升高,差异具有显著性(P＜0.05).结论:益气化浊利水方联合西药能有效改善HRV并降低NT-ProBNP,减轻改善慢性左心功能不全患者心功能不全症状.%Objective:to study the effect of Yiqi Huazhuo Lishui Formula (formula for replenishing qi,resolving turbidity and excreting water) on heart rate variability (HRV) and changes of N-terminal Pro-brain natriuretic peptide (NT-proBNP) in patients with chronic left ventrieular dysfunction.Methods:Patients with chronic left ventricular dysfunction who meet the inclusion criteria were randomly divided into treatment group and control group.Control group were treated with diuretics,nitrates and angiotensin converting enzyme inhibitors,while treatment group with Yiqi Huazhuo Lishui Formula in addition to conventional western medicine.HRV of both groups were detected by dynamic electrocardiogram before treatment and after 4 weeks of treatment,including SDNN,SDANN,RMSSD,and PNN50,and NT-ProBNP was detected by blood test.Results:NT-proBNP of both groups after treatment decreased (P ＜ 0.05),SDNN,SDANN,RMSSD,and PNN50 increased;compared to control group,NT-proBNP of treatment group decreased significantly while SDNN,SDANN,RMSSD,and PNN50 increased significantly,with significant differences (P ＜ 0.05).Conclusion:Yiqi Huazhuo Lishui Formula combined with western medicine can effectively improve HRV and reduce NT-ProBNP,and relieve the symptoms of cardiac dysfunction in patients with chronic left ventricular dysfunction.
