舒血宁注射液辅助治疗对 COPD 急性加重期患者 TLRs 所介导炎症反应的影响

             

摘要

[ABSTRACT]Objective:To study the effect of adjuvant Shuxuening injection therapy on the inflammatory response media-ted by TLRs in patients with acute exacerbation of COPD.Methods:Patients with acute exacerbation of COPD who received Shuxuening injection combined with conventional western medicine treatment in our hospital between May 2014 and December 201 5 were selected for study,and before treatment as well as 1 week,2 weeks,3 weeks and 4 weeks after treatment,peripher-al blood mononuclear cells were collected to determine TLRs expression,and serum was collected to determine the levels of in-flammatory cytokines and airway remodeling molecules.Results:At 1 week,2 weeks,3 weeks and 4 weeks after treatment, TLR2 and TLR4 mRNA expression in peripheral blood mononuclear cells as well as TNF-α,IL-6,MCP-1,VCAM-1,MMP2, MMP9,TGF-βand BMP-2 levels in serum of patients with COPD were significantly lower than those before treatment,and TLR3,TLR7,TLR8 and TLR9 mRNA expression were not significantly different from those before treatment;TLR2 and TLR4 mRNA expression in peripheral blood mononuclear cells were positively correlated with serum TNF-α,IL-6,MCP-1, VCAM-1,MMP2,MMP9,TGF-βand BMP-2 levels.Conclusions:Adjuvant Shuxuening injection therapy has significant in-hibitory effect on the inflammatory response mediated by TLRs in patients with acute exacerbation of COPD.%目的::研究舒血宁注射液辅助治疗对慢性阻塞性肺疾病(COPD)急性加重期患者 Toll 样受体(TLRs)所介导炎症反应的影响。方法:选择2014年5月~2015年12月期间在我院接受舒血宁注射液联合常规西医药物治疗的 COPD 急性加重期患者进行研究,治疗前和治疗后1、2、3、4周时采集外周血单个核细胞并测定 TLRs 表达量,采集血清并测定炎性因子、气道重塑分子的含量。结果:治疗后1、2、3、4周时,COPD 患者外周血单个核细胞中 TLR2、TLR4的 mRNA 表达量以及血清中肿瘤坏死因子(TNF)-α、白介素(IL)-6、单核细胞趋化因子-1(MCP-1)、血管细胞黏附分子-1(VCAM-1)、基质金属蛋白酶(MMP)2、MMP9、转化生长因子(TGF)-β、骨形成蛋白(BMP)-2的含量均显著低于治疗前, TLR3、TLR7、TLR8、TLR9的 mRNA 表达量与治疗前比较无显著性差异;外周血单个核细胞中TLR2、TLR4的 mRNA 表达量与血清中 TNF-α、IL-6、MCP-1、VCAM-1、MMP2、MMP9、TGF-β、BMP-2的含量呈正相关关系。结论:舒血宁注射液辅助治疗对 COPD 急性加重期患者 TLRs 所介导炎症反应具有显著抑制效应。
