目的探讨使用套囊充垫法来提高经鼻明视下加强型导管气管插管准确度的可行性。方法选择80例需行经鼻气管插管全麻的患者，ASAⅠ～Ⅱ级，无经鼻插管禁忌征及困难插管表现，随机分为A、B两组，每组40例，均快速诱导后以加强型气管导管明视下插管，其中A组在插管过程中应用套囊充垫法， B组采用传统方式的直接推送导管入声门，所有患者两次插管未成功则以插管钳辅助完成插管。比较两组患者一次插管成功率、两次总的成功率、使用插管钳的比例及插管时间。结果 A组一次插管成功率和两次总的成功率显著高于B组，使用插管钳的比例显著小于B组，A组插管时间显著少于B组。结论在加强型气管导管经鼻明视下气管插管操作中，与传统方式相比，套囊充垫法可显著提高插管准确性，缩短插管时间。%Objective Explore the feasibility of reinforced tracheal tube cuff inflation as aid to improve the accuracy in nasotracheal intubation under photopic vision. Methods Select 80 cases of patients undergoing nasotracheal intubation general anesthesia, ASAⅠ- Ⅱ ,without nasotracheal intubation contraindication and difficult airways. Patients were randomly divided into A, B groups, 40 cases in each group,intubated with reinforced tracheal tube under photopic vision after rapid-sequence induction. Cuff-inflation method was applied in Group A during intubation,and the traditional method of direct push tracheal tube into the glottis is used in group B.If tracheal intubation was not successful two times ,then complete intubation by intubation forceps.Compare the two groups of the first success rate, total success rate of two times,the proportion of using intubation forceps,the endotracheal intubation time.Results The first success rate, total success rate of two times of group A were significantly higher than those of group B.The proportion of using intubation forceps and the endotracheal intubation time of group A were significantly less than those of group B.Conclusion In nasotracheal intubation under photopic vision with reinforced tracheal tube ,compared with the traditional way,cuff-inflation method can significantly improve the accuracy of intubation, shorten the time of the endotracheal intubation.
